Help my fishey!

I got this fish along with several others. This is the only guy who had white spots. The previous owners told me that it was from rubbing up against the rough rocks they had in there.

A few weeks after I got these guys this one was having trouble swimming. He could control himself, he just looked relaxed, (sort of like Dori from little nemo). Swimming kind of diagonal.

I took him out and put him into a 20 gallon long hospital tank with a filter (bio wheel) and heater.

The next part is out of order, as this has been going on for months and everything I have tried has been a blur:
He started getting white fluffy stuff coming out of holes in his head. Later he had open red sores. Another time he had almost pimples, red around the edge, white at the head.

I tried weeky water changes, daily water changes, perfect conditions, imperfect conditions, salt, warm water...

I've tried fungus cure, melafix, pimafix. Those didn't do much. I tried EM Erythromycin which sort of worked. I tried rid-ich+ which has worked the best.

The problem is a few days into the treatment even though his skin got better he started acting worse. I gave up and stopped for a week or so.

I then startedup the rid-ich+ again. I went for it. Do-or-die. Then he stopped eating and I quit with the rid-ich. Now he's not eating, and not really swimming.

The pictures are of him today. Ignore the white on his sides, that is what he came with, which has never changed in the many months I have had him. Problem is his eyes for the first time now are cloudy, and he doesn't care if I touch him, push him, poke him. His skin problems have been mostly cleared by the rid ich, but I'm supposed to use it longer...

HELP! I've tried more OTC medicines than I can remember. Something's gotta give here!
